The Care Center will cater to Medicare beneficiaries in Kings County and their caregivers, offering services such as podiatry, diabetic education and management, fall prevention, blood pressure monitoring and more. The center includes comfortable and safe exam tables, lightweight doors, and low-glare accessories. It also has a gym-like area with equipment devoted to strengthening exercises as well as classrooms for socializing and learning. The Care Center will be available exclusively to members of certain Empire Medicare Advantage plans. | |

Empire BlueCross BlueShield is a Health plan with a Medicare contract. Empire HealthChoice HMO, Inc.
