CHICAGO--()--Presence Health and its Accountable Care Organization (ACO) have signed an agreement with Emmi Solutions, the leader in outcomes-driven patient engagement to roll out Emmi’s comprehensive, interactive, multi-modal platform throughout its entire 12-hospital system and ACO. Emmi Solutions delivers information to patients and empowers them to take action around health events or conditions at key points across the continuum of care.

Emmi’s platform was first introduced to the Presence Health system at Presence St. Joseph Hospital in Chicago. Ninety-seven percent of patients who had an Emmi experience said questions were answered for which they normally would have called their doctor, and 80 percent said it increased their confidence to ask their doctor questions. In addition, 70 percent said it improved their opinion of the hospital.

The Emmi® platform allows healthcare professionals to “prescribe” Emmi programs to patients who can view at their convenience, on their computer or mobile device, and share the program with friends and family. The information provided helps patients make sense of complex medical information as well as manage expectations around procedures and chronic health conditions.
