Designed for health care leaders and professionals working in primary care and community settings - from beginners in improvement to experienced veterans in the field - the Summit will focus on new ways to engage patients and communities to improve health status, new approaches to implementing and spreading patient-centered medical homes (PCMH), and new opportunities for leveraging health IT, among other topics. |
