NCQA is a private, non-profit organization dedicated to improving health care quality. The NCQA fully accredited the following chronic conditions: Asthma; Chronic Kidney Disease; Congestive Heart Failure; Chronic Obstructive Pulmonary Disease (COPD); Coronary Artery Disease; Diabetes; Hypertension; Osteoporosis; and Rheumatoid Arthritis.
